Plastic Surgery Patient Intake

Thank you for contacting our office regarding your plastic surgery needs.

We would love the opportunity to discuss with you the surgery options Dr. Salvador Medina would recommend for you.

In order to accurately evaluate and quote you on the surgery/surgeries you may need, we ask that you send pictures so that the surgeon can assess your individual needs.

If you are looking for body contouring, you can send us pictures from the neck down. We will require 1 front, 1 back and 1 side profile picture, and please make sure to not wear a bra for any surgery evaluations and no and/or small underwear for a lower body evaluation.

If you are looking for a face evaluation; please send 1 face forward picture and 1 side profile picture.
